REPORTING REQUIREMENTS: 2009 Program Reporting Guidelines and Forms

2009 Program Reporting Guidelines and Forms

On this page you will find guidance for the completion of the Semi-Annual Progress Report . Your progress report must describe both programmatic and fiscal activities. The first semi-annual progress report is due no later than 5:00 p.m. on March 30 of the funded year and the second semi-annual progress report is due no later than 5:00 p.m. October 30 of the funded year. The document should be submitted to Jan Brite, Bin #60 Arizona Department of Education, 1535 W. Jefferson Phoenix, AZ 85007.

The Match Verification Form #2 must accompany your Semi Annual Report. It is essential that you show that you met your 50% of the total program cost (dollar for dollar) match on the Match Verification form.

Please Note: Required reports and deadlines are based on the requirements and deadlines of the Corporation for National Service, the funding source for the Learn and Serve Grant. If requirements change or additional information is required by the Corporation for National Service, it will be necessary to adjust state reporting accordingly. It is crucial that you meet the requirements and deadlines in order for the Arizona Department of Education to meet the deadlines set by the Corporation for National Service.
